Yes. If you’d like to pay by credit or debit card I will email you an authorization form which will allow you to securely pay via credit or debit card on-line. I use a company that provides bank-level security for these transactions.
I do not take insurance directly, however, I will provide you with a monthly statement that you can submit to your insurance company for reimbursement. Many PPO plans will reimburse a portion of the fee to out of network providers. HMO plans generally do not reimburse. Insurance plans vary with regard to whether or how much they will reimburse, so it’s best to call you insurance carrier and ask them for details.

Please call or email me as soon as you know that you can’t make an appointment. I have a 24 hour cancelation policy. If you cancel more than 24 hours before the appointment you won’t be charged. If you cancel with less than 24 hours’ notice you will be charged for the time. If possible, I will try to reschedule you to a later time during the week so you don’t have to pay the cancellation fee, but I cannot guarantee this.
